Older adulthood should be a time for enjoyment and celebrating much-deserved independence. Sometimes, however, aging creates challenges that can be a barrier to this independence.

For over 20 years, the Butler County Elderly Services Program (ESP) has provided home-based services that can assist with aging-related challenges and help to ensure continued independence. ESP is funded by a countywide tax levy and is administered by the Council on Aging of Southwestern Ohio. LifeSpan has partnered with ESP since its inception and provides the Care Management as well as the Intake and Information and Referral for the program.

If you or a loved one are seeking information on services or community resources, please call our Information and Referral line at (513) 868-9281.  LifeSpan’s Information Specialists have over 40 years of combined experience with extensive knowledge of local resources.  Specialists are Community Resource Specialists – Aging/Disabilities. 

Yes, we do offer a private pay care management service.  Family Connection serves as a liaison between the older adult, family members, doctors, and other resource providers by coordinating communication so that everyone is informed and updated at all times. We provide more than a service. We create a professional relationship that helps older adults stay in their own homes for as long as possible while maintaining a high quality of life.

No.  These services are considered non-skilled, which means they are not medical in nature and do not require a physician’s order.  Elderly Services Program does not provide skilled care such as a nurse to check vitals or fill medications, physical therapy or occupational therapy.  
  1. Age: Eligibility is first based on age. For Home Delivered Meals, you must be at least age 60. For all other services, you must be at least age 65.
  2. Location: It is based on residency.  You must reside in Butler County. 
  3. Deficits: You must have two deficits related to the tasks you complete each day in order to live independently. These include activities such as cleaning, driving, eating, shopping, bathing, ambulating, etc.
  4. Need: You must have deficits relative to the service you are requesting. For example, if you are able to drive and have consistent access to transportation, you would not be eligible for medical transportation. However, if you have vision impairments and no longer drive and need assistance getting to your doctor’s appointments, you would likely be eligible for medical transportation.

Cost will vary for each individual. Care Managers will verify your monthly income as well as monthly medical expenses that you incur (i.e. medication costs, insurance costs, hospital bills, etc.). We then deduct your medical expenses from your income and use the remaining balance as your countable income.

We have a sliding fee scale to determine whether or not you have a co-payment for your services. Your co-payment is based on the total cost of the services you receive through ESP.

Example: You receive 2 hours of homemaking each week and an Emergency Medical Response System (lifeline button). The total cost of these services is $200 per month. If you have a 10% co-pay, you would only pay $20 per month for your services.

  1. Call our Intake Department at 513-868-9281 or fill out our online referral form.
  2. You will participate in a telephone assessment which typically takes between 30-45 minutes.
  3. Once the assessment is complete and you have been determined eligible for the program, your information will be given to a Care Manager.
  4. Your Care Manager will then call you to schedule a face to face home visit.
  5. At this home visit, the Care Manager will review the information you provided during the telephone call as well as gather some additional information and establish your care plan.
  • The Care Manager will review the information you provided during your telephone assessment as well as collect some additional information.
  • They will ask to visually verify your income and medical expenses.  You should be prepared to show proof of income which could include a Social Security or pension statement, a bank statement, your check register, etc.
  • You should also be prepared to show proof of monthly medical expenses.  These could include your check register, receipts, a print out from the pharmacy, a statement, etc.
  • Based on this information, your Care Manager will be able to tell you whether or not you will have a co-pay for services.
  • You and your Care Manager will discuss the services you are in need of and will establish a care plan.
  • You will also be asked to sign a couple of forms. 
Typically you will receive a call to schedule your home visit in about two to three days after you complete your phone assessment. You and your care manager will then schedule your home visit, usually within the next five days. The start of services depends upon the service. Generally, your services start within a couple of weeks from your home visit.

Katie has been with LifeSpan since 2010. She graduated from the University of Cincinnati in 2006 with a Bachelors degree in social work and from the University of Alaska in 2009 with a Masters degree in Social Work. She is certified a Licensed Social Worker through the State of Ohio, an Advanced Social Work Case Manager through the National Association of Social Workers and an Aging Life Care Professional through the Aging Life Care Association. Katie has experience in the field of aging, child welfare, and victim advocacy. In her free time, Katie enjoys spending time with family.
