The registered nurses and social workers who staff the Intake & Screening (I&S) service at LifeSpan provide the single point of entry to the Butler County Elderly Services Program (ESP). With one telephone call, older adults or others who care for them can begin the process of determining whether they are eligible to receive ESP services which can help them remain safe and independent in their own homes.

• Initial telephone screening.
• Referrals for eligible clients to an ESP care manager based in or near their specific community who then arranges a face-to-face meeting to determine the services needed.
• Screening of potential clients who are referred by physicians, hospitals, nursing homes, and other agencies.
• Referrals for eligible clients to other aging services as necessary.
