LifeSpan’s Family Connection provides
safety and stability for those who wish to
remain living independently and peace of
mind for their families.
Family Connection provides the link
between older adults, services they need,
and those who care for them, typically
their grown children or other loved ones.
The program’s professional care managers,
either a nurse or social worker, evaluate the
situation then recommend and coordinate
needed services. The care manager makes
home visits, can accompany a client to
medical appointments, maintains contact
and provides written updates to family
members. Family Connection services are
fee-based and can be designed on a shortterm
or ongoing basis.
