Elderly Service Program

The Butler County Elderly Services Program (ESP) provides a wide range of services for older adults who need assistance to continue living independently in their own homes. Care managers at LifeSpan evaluate the needs of ESP clients, set up services and monitor them on an ongoing basis. LifeSpan care managers are social workers, registered nurses or bachelor’s prepared professionals and are contracted through the ESP program which is funded by the county tax levy and administered by the Council on Aging of Southwestern Ohio.

Care managers troubleshoot new issues as they arise and act as a liaison between their clients and other organizations. They often make referrals to community agencies and continue to monitor clients’ needs while services are in place.
