Job Description:
We are currently seeking a Elderly Services Care Manager (CM) to join our Elderly Services Program. As a Care Manager you will provide the service of initial and on-going assessment of clients’ needs and eligibility for ESP services, will develop a care plan based on these needs, identify options and see to the facilitation and coordination of services to meet those needs through the use of contracted Providers and other available resources to promote quality, cost-effective outcomes. CMs will also provide client advocacy as needed. Specific responsibilities include the following:
• Assess clients’ program eligibility, provide information, make necessary service referrals
• Develop individualized service plans based on assessments and reassessments in collaboration with client and/or caregiver to effectively and efficiently meet client needs
• Monitor use and delivery of service providers to assure the health and safety of clients as well as contract compliance
• Maintain required contact with clients, service providers and others involved to ensure care plan is needs are met
• Solve any problems in service delivery and assess for any changes in client status, conditions or needs, provide support and coordination as needed
• Maintain accurate, confidential and up to date records as required by established policies and procedures
• Attend required program, agency and case specific meetings
Requirements:
• Must be a Registered Nurse or Licensed Social Worker, graduate of an accredited school and licensed in the State of Ohio, or bachelor’s prepared degree in social work, gerontology or related field
• Must have a valid driver’s license and reliable transportation
• Must have an acceptable criminal records check as defined in the ODH criminal records check rule 3701-61-10.
• Ability to effectively communicate orally and in writing

LifeSpan is seeking a full-time outpatient therapist to work with their time divided between our Hamilton and Middletown offices. This person will provide counseling services to individuals, couples, family or groups as referred by other agencies or community programs.
Duties include:
1. Providing direct service to clients including individual, couple, family and/or group therapy; coordinating and/or conducting educational programs; and coordinating other services as needed by the client.
2. Maintaining individual client records as directed by the Clinical Manager; submitting reports requested by division/program managers; compiling client and group progress notes, summaries, etc.
3. Attending division and agency staff meetings, appropriate community meetings, and workshops, conferences and seminars related to primary job descriptions
4. Attending case conferences regarding clients
5. Promoting a positive image of the Agency in the community
6. Other responsibilities as designated by the Clinical Manager, FLS Director or the CEO.
7. Participating in the PQI process as directed by the Clinical Manager and FLS Director.
Requirements:
Masters’ Degree in Social Work or Counseling required. LSW or PC licensure only need apply. LISW or LPCC preferred.
